Close accounts you know have been tampered with or opened fraudulently.
Change online passcodes.
File a police report. Get a copy of the report to submit to your creditors that may require proof of the crime.
If you have disclosed sensitive information in a phishing attempt or some other type of scam, you should also contact one of the three major credit bureaus and discuss whether you need to place a fraud alert on your file. This may help prevent thieves from opening new accounts in your name. Here is the contact information for each bureau's fraud division:
